Advancing Community Health Across the United States

Building Healthier Communities Through Accessible Infectious Disease Care

Our integrated model brings care directly into communities, reduces barriers to treatment, and improves long-term health outcomes for underserved populations.

Our Mission

LifeLine Health works to expand access to high-quality infectious disease care for underserved and high-risk populations through community-based testing, treatment, and prevention programs focused on Hepatitis C, HIV, and other sexually transmitted infections.

By partnering with healthcare providers, public health agencies, universities, and community organizations, LifeLine Health develops practical, scalable care models that improve early detection, increase treatment completion, and strengthen public health infrastructure.

Our work is grounded in the belief that effective infectious disease prevention and treatment require trust, accessibility, and strong collaboration with the communities being served.

The Lifeline Health Model

LifeLine Health programs operate through an integrated community-based care model that combines testing, treatment, prevention, and patient support services to remove barriers to care and improve long-term health outcomes.

Testing

Accessible, confidential screening for HIV, Hepatitis C, and other sexually transmitted infections delivered through community clinics and healthcare partners.

Treatment

Evidence-based treatment pathways that help patients begin and complete therapy, including curative treatments for Hepatitis C and coordinated HIV care.

Prevention

Education, outreach, and prevention strategies that reduce transmission and support long-term community health.

Support Services

Patient navigation and care coordination that address financial, social, and logistical barriers to care.

This integrated approach improves linkage to care, supports treatment completion, and strengthens long-term health outcomes.
